"What do you want?" is a general question.

What do you want to do this weekend?

Shop Assistant: What would you like to buy?
Customer: I'd like to buy a pair of shoes.
Shop Assistant: What size are you? What size do you wear?

"Which do you want?" implies that there is a limited or defined selection to choose from.

Customer: I'd like to buy a sweatshirt.
Shop Assistant: They come in small medium and large. Which size would you like?

"Which" can be translated as "lo cual".

My car broke down which means I'm gonna have to take the train to work.
